There are lots of stances one can take when reading. You can do a close reading way down low on hands and knees. You can sprint over the surface, skimming and dipping down every once in awhile. Or you can levitate over the reading field at various altitudes. I did a combination of these recently in my reading of The Art of Is by Stephen Nachmanovitch. Here is the map of that.
Close reading: cut out words from text and positioned in a rough poem around the verge, rethinking close annotation by adding directly to the text annotations with callouts and other text boxes.
Above the fray: notes on using the browser, images, video. Also, notes on the limits of the tool and rethinking the white space and meta work on hacking/pirating the space.
